PHCC Rolls Out Case Management Services in all Health Centers

Doha, August 09 (QNA) - Primary Health Care Corporation (PHCC) has rolled out the case management services for patients suffering from chronic diseases in all its health centers as part of the second phase of the family medicine model.

After the successful pilot program for case management services in the health centers of Umm Salal, West Bay, Al Wakrah and Al Wajba, PHCC expanded the service scale to encompass 27 health centers from the remaining ones.

In addition, PHCC stated that the provision of case management services is a vital accomplishment, because such services enhance the integrated Family Medicine Model of care, as such service is a globally recognized model which has proven its effectiveness in health care systems in all over the world.

The service has been specifically designed to provide overarching and persistent care to patients suffering from multiple chronic diseases who need complex care.

At the core of the case management services, there is an integrated model for care featuring a multidisciplinary team from PHCC's specialists, who are collaboratively working to provide subjective plans, care coordination and case management to ensure patients receiving an inclusive care that revolves around patients to optimize health results, especially for persons suffering from concomitant diseases.

Executive Director of Operations at PHCC Dr. Samya Ahmad Al Abdulla said the activation of case management services reflects PHCC's commitment to upgrading health care services in primary health, adding that during the pilot phase there has been an improvement in patients' experience which led to better health results, improvement of their life quality, reducing the need for services and integrated care and adopting an approach of more efficient integrated care in a cost-effective way.

She outlined that the benefits patients will gain from the case management services entail the optimization of persistent care, reducing inessential visits to hospitals, ensuring appropriate care for patients, and managing their chronic diseases more effectively.

The expansion of family medicine model through operationalizing the case management services is a vital step in achieving PHCC's vision which is primarily based on providing an effective preventive and personal primary care in the atmosphere that surrounds patients.
